Why occipital neuralgia keeps firing, what dural tension has to do with it, and why most approaches never reach the real problem
It starts at the base of your skull. Sharp. Burning. Sometimes electric. It wraps up and over, and some days it feels like your entire head is under siege. You've been told it's tension. You've been told it's stress. You've tried the muscle relaxers, the nerve blocks, the ice packs. And maybe some of it helped for a while. But it keeps coming back.
A quick look at what is inside your guide.
Sharp or Shooting Pain at the Base of the Skull
Sudden, electric-like pain that starts at the occiput and radiates upward, often on one side.
Burning Sensation Across the Scalp
A persistent burning or aching that spreads from the base of the skull toward the top or sides of the head.
Pain Behind the Eyes
Referred pain that travels forward from the occipital region, sometimes mimicking a migraine or sinus headache.
Tenderness at the Base of the Skull
The suboccipital region feels tight, sore, or painful to the touch, often on both sides.
Light Sensitivity or Visual Disturbance
Sensitivity to light, blurred vision, or a feeling of pressure behind the eyes during flare-ups.
"The occipital nerves don't fire in a vacuum. The dura mater is directly adherent to the occiput, and when it's under sustained tension, it pulls on the very nerves that serve the back of your head. Most approaches treat the nerve. We evaluate the dural tension driving it."
Here's what most people are never told: occipital neuralgia isn't just a nerve problem at the back of your head. It's often driven by tension in a structure most providers never evaluate. The dura mater, the tough protective membrane surrounding your brain and spinal cord, is directly adherent to the inside of the occiput, the bone at the base of your skull. When that membrane is under sustained contracture, it pulls tension directly onto the nerves that exit at the occiput. That's the conversation we want to have with you.
